Community Centered Services

 

Turning Point's Community Centered Services (CCS) programs provide therapeutic interventions and support to area youth and families.  These services are client centered, family focused, and community based. 

 

The programs focus on treating youth and families in the community.  The goal is to either prevent more intensive services or to allow for gradual ‘step down’ in intensity from higher levels of care.  Consistent support and gradual community reintegration has been found to decrease recidivism and improve independent performance. 

 

A primary focus is to properly assess the youth and families individual needs and place them in the level of service that is the least restrictive and most appropriate to ensure success.  These services include:  individual, group and family therapy; in-home therapies; substance abuse treatment; tracking services; aftercare follow-up; mentoring; foster parent consultation, support and training; and coaching.

  • Crisis Intervention Services

 

Turning Point offers referral, assessment, counseling, and treatment to children ages 4-23 and their families who are in crisis due to behavioral health and/or substance abuse issues. Turning Point helps families in crisis by providing the following:

 

  • Referral to services that most closely meet your child’s needs
  • Individualized treatment plans
  • In-home therapies and treatment
  • Psychiatric consultation
  • Case Planning
  • Behavioral health and substance abuse treatment through individual, group and family therapies


With many thanks to The Daniels Fund and the United Way of Larimer County, these services are free to qualified youth and families.

 

 

 

 

  • In-Home Therapies

 

Turning Point’s In-Home Therapy captures key components from multiple Evidenced-Based Best Practices.  Upon receiving a referral, the youth, family, referral agent, and the clinician all meet and identify what changes they all agree to work toward.  The focus is not only to identify areas for change, but also to get all the participants on the same page to ensure continuity of care.

 

From this meeting, a strength-based treatment plan is developed focusing on short and long term goals and identifying barriers to achieving those goals.  The plan is revisited each session to identify which short term goals have been accomplished, what barriers were identified that kept the client from accomplishing certain goals, and identifying new goals.  By continuously revisiting and updating the treatment plan, both the clients and therapist stay focused on the stated goals. 

 

CCS is staffed with Masters' level therapists who provide individual, group and/or family therapies when appropriate and part of the case plan for a youth and/or the family. Our therapists are trained in Motivational Interviewing, Motivational Enhancement Therapy, Dialectical Behavioral Therapy, Cognitive Behavioral Therapy and Play Therapy.  In-home therapy is provided on a fee-for-service basis and billed to the referring agency, insurance companies, and/or the client.

 

 

  • Foster and Group Home Parent Consultation

 

Turning Point provides therapeutic consultation to Foster and Group Home Parents dealing with children placed in their care who are displaying maladaptive behavior issues. The goal of the program is to teach Foster and Group Home Parents the skills they need and provide ongoing support to maintain Foster placements in the home.

 

Turning Point therapists communicate with the caseworker and other professionals involved to provide assessments, clinical recommendations and opinions in order to present a unified Social Services response for court.  Turning Point staff is trained and familiar with all reporting and confidentiality laws, and therefore assure compliance in this area. 

 

 

 

 

  • Family Care Coordination

 

This program is collaboration between Turning Point, Larimer Center for Mental Health, and The Matthews House.  Family Care Coordinators are Masters' level therapists who provide solution focused therapy and case management to youth and families referred from Larimer County Human Services.  FCC's also collaborate with other treatment providers working with the family to assure cohesive treatment and continuity of care.  This innovative cross-agency program utilizes one clinical supervisor and collective training to assure fidelity to the treatment model.

 

 

 

 

  • Coaching

 

This service was developed in collaboration with Larimer County Department of Human Services using the Hampton, Virginia Model. Coaching is designed to provide support, structure, positive role modeling, mentoring, and life skills to youth residing in the community.

 

This service is designed to be very flexible in meeting the individual needs of the youth and family and can include recreational activities, transportation, teaching in home skills such as cleaning and organization, attendance at professional meetings or appointments, etc. Our coaches consist of a variety of part-time and full-time staff personnel from Turning Point and the community at-large.

 

 

 

 

  • Mentoring

 

Turning Point offers mentoring services through a contract with Matthews House in Fort Collins. This service offers an opportunity for youth to develop positive relationships with healthy adult role models within their home community and assists youth in developing positive relationship skills.

 

 

 

 

  • Outpatient Therapy

 

Community Centered services is staffed with master's level therapists who provide individual, group and family therapies when appropriate. Our therapists are trained in Motivational Interviewing, Dialectical Behavioral Therapy, Cognitive Behavioral Therapy and Play Therapy.

 

 

 

 

  • Tracking Services

 

Tracking Services provide for youth living within the community, many of whom are on parole. These services include client monitoring; life skills training; school enrollment and monitoring; job search assistance; housing assistance; curfew checks, home visits and inspections; UA/BA monitoring; individual and/or family therapy; mentoring; and Electronic Home Monitoring.

 

This service generally is applied as a package and based on number of contacts needed per week or month. At times youth only need one or two very specific services so we offer these services individually as well.
